Creating a Patient Portal Account

• In order to create your log in to your patient portal, you will be mailed or handed at the time of your appointment, a Portal Letter. This letter will have your log in and password
• Click on the Medent logo at the top of the page.
  
• Log in with the information provided in the letter. (You will be able to change your log in and password once you are in the patient portal.

What Can I Do in My Patient Portal?

• Request test results
  
• Contact staff through triage
  
• Contact the office to request/change an appointment
  
• Request a prescription refill
  
• Update medications
• Update personal information
• Get your current scheduled appointment information
